Filling out the Affidavit Parental Rights form is an important step in the process of relinquishing parental rights. After completing the form, you will need to submit it to the appropriate authority, usually a court or notary public, to ensure it is legally recognized.

  6. Identify the mother and legal guardian of the child, including her name and the child’s name.
  7. Choose between option 5A or 5B by placing an X in the appropriate box and completing the statement regarding child support obligations.
